You can ask us to delete your GoTCMHUB app account at any time. You do not need the app installed, and you do not need to sign in to make this request.
How to request deletion
Call the front desk — fastest, and we can confirm it while you are on the line.
Or email services@tcmhub.org with the phone number on your record. We reply within 2 business days and complete verified requests within 30 days.
We verify your identity before deleting anything. This protects your record from being deleted by someone else.
Still have the app? You can delete your account yourself: Settings → Delete account.
What is deleted
Permanently removed, and cannot be restored:
- Your app sign-in
- All active sessions, on every device
- Your notification tokens — the app stops sending you anything
- Your marketing contact permissions
- Your member code
What we must keep, and why
As a licensed California healthcare provider, TCMHUB is required by law to retain your medical record — your visit history and treatment notes — for a minimum period after your last visit. We cannot delete those records at your request, and we do not use them to contact you.
This is a legal retention obligation, not a copy of your app account. Deleting your app account does not close your patient record at the clinic, and it does not affect any appointment you have already booked.
If you want to use the app again later, register as a new user.

或來信 services@tcmhub.org， 並附上您建檔時的手機號碼。我們會在 2 個工作天內回覆，並在 30 天內完成已驗證身分的請求。
我們會先驗證您的身分才進行刪除，以免您的資料被他人刪除。
App 還在手機上？您也可以自行刪除：設定 → 刪除帳號。
會被刪除的
永久移除，無法復原：
- 您的 App 登入
- 所有裝置上的登入狀態
- 您的通知權杖——App 不會再向您發送任何通知
- 您的行銷聯絡同意
- 您的會員碼
依法必須保留的，以及原因
本院為加州合法執業之醫療院所，依法必須在您最後一次就診後保留病歷——包含就診紀錄與治療記載——達法定期間。我們無法依您的要求刪除病歷，也不會用它來與您聯絡。
這是法定保留義務，不是您 App 帳號的副本。刪除 App 帳號不會結束您在本院的病人身分，也不影響您已經預約的門診。
日後若想再使用 App，重新註冊即可。
